Wanting the blank leaf Z12.
Binding: Contemporary German blind-tooled pigskin over bevelled wooden boards, clasps and catches lost; four parchment index tabs. On the upper cover quadruple fillets form a triple frame; within the outer frame a lozenge-shaped pomegranate stamp; within the second frame a circular lamb-and-flag stamp, lozenge-shaped eagle stamp, and an octagonal rosette stamp; the inner rectangle is divided by diagonal triple fillets into four triangular compartments, each with a square stamp with S. Catherine, a fleur-de-lis stamp, and the octagonal rosette stamp. On the lower cover quadruple fillets form an intersecting double frame; within the outer frame a lozenge-shaped foliate stamp at the four corners, the lamb-and-flag and the floral stamps on the sides; the inner rectangle is divided by quadruple fillets into triangular and lozenge-shaped compartments. Title and paper labels on the spine. Title across the upper edge:`Decretum'; across the fore-edge: ‘Distinctiones centum et una', ‘Cause 36', ‘De pe[nitentia] di[stinctiones] 7', ‘De conse[cratione] di[stinctiones] 5.’

Front pastedown, front endleaf, back endleaf, and back pastedown (the latter now mostly torn away, while an impression of text left on the back board) consists of four mid-thirteenth-century manuscript parchment leaves containing Boethius, De Divisione, with an interlinear gloss; see Margaret T. Gibson, Lesley Smith, and Joseph Ziegler, Codices Boethiani, I (London, 1995), 183 no. 168.
On a1r early notes in German; upper left: ‘Quare Lutherus ius canonicum combusserit, habes 2 to[mus] operum M[artini] L[utheri] p. 119 a, Latina edit[ione] anno 62 Wittenb[ergensis]'; middle right: ‘D. Luth[er] 378 b. serm[ones] conui[uales]. Decretum ist ein zusamen gerafft ding, wie ein betlers mantel zuhauff geflikht, von vilen haderlumpen, ein jedlicher theologe soll sie lesen, doch cum judicio. 1 Thes. 5. plura ibi 379 a'; lower portion: ‘Luther. Geistlich recht ist ein shendlich buch, das nach geld stinkht, nim[m] daraus ehrgeitikeit v[nd] geitz, so wird nichts drinnen bleiben von seinem wesen.' Early marginal annotations, mostly extracting key words.
On a2r an eight-line initial is supplied in blue edged in green with red pen-work decoration. Other three-, four-, and six-line initials are supplied in blue, some with red pen-work decoration.
